Dixon Hearts Group Home is licensed by the Agency for Persons with Disabilities (APD). We provide a comprehensive range of services to individuals with developmental disabilities. These services are primarily funded through Florida’s iBudget Medicaid Waiver, which supports community-based living and individualized care.

Core Services Offered by Dixon Hearts Group Homes 

Through(APD) services.

    1.    Residential Habilitation

    •    24/7 support with daily living activities such as bathing, dressing, and meal preparation.

    •    Supervision to ensure safety and well-being.

    •    Assistance with medication management and healthcare coordination.

    2.    Behavioral and Mental Health Support

    •    Access to specialized mental health counseling tailored for individuals with developmental disabilities.

    •    Implementation of behavior support plans to address specific needs. 

    3.    Medical Oversight

    •    Annual medical reviews conducted by licensed nurses to ensure appropriate healthcare follow-up and medication management.  

    4.    Community Integration

    •    Support for participation in community activities, social events, and recreational outings.

    •    Assistance with transportation to appointments and community engagements.

    5.    Skill Development

    •    Training in daily living skills, communication, and social interactions to promote independence.

    •    Support for employment opportunities and vocational training.

Eligibility and Application Process

To be eligible for APD services, individuals must:

    •    Be a Florida resident.

    •    Have a documented developmental disability (e.g., intellectual disability, autism, cerebral palsy, Down syndrome).

    •    Apply through the APD’s online application system.  

The application process includes providing documentation of the disability and may involve assessments to determine the level of need.
